Position Overview
As a Night Clinical Pharmacist, you will deliver comprehensive pharmaceutical care to assigned patients under the guidance of a senior clinical pharmacist or manager.


Your role involves evaluating and interpreting physician orders, dispensing medications, and providing pharmaceutical supplies in accordance with established operational protocols, professional standards, and relevant legal regulations.

Key Responsibilities
1. Assess medication orders for precision and completeness.

2. Address any discrepancies in medication orders through effective communication with physicians, clinical pharmacists, or nursing staff, and ensure accurate dispensing of medications.
3. Evaluate the feasibility of compounding and verify the preparation of extemporaneous products.

4. Suggest alternative medications when necessary.

5. Prepare intravenous admixtures and compound sterile products, including large volume parenterals, mini-bags, TPN solutions, and hazardous agents such as cytotoxic drugs.

6. Respond independently to non-evaluative drug information inquiries.

7. Monitor and take appropriate actions to optimize patient drug therapy, ensuring effective drug selection and minimizing adverse drug reactions under the supervision of a senior clinical pharmacist or manager.

8. Review patient profiles to identify contraindications related to prescriptions.

9. Screen medications and dosages to ensure appropriateness based on therapeutic intent and the patient's physiological status.

10. Participate in monitoring drug therapy under the supervision of a clinical pharmacist in decentralized settings.

11. Dispense controlled substances while maintaining an accurate inventory, ensuring compliance with legal requirements and departmental policies.

12. Provide technical oversight to support staff.

13. Supervise workflow and attendance to promote efficient operations.

14. Verify the accuracy of technicians in medication preparation and packaging.
15. Enter and validate computer entries and data for drug orders. In decentralized settings, you may also educate patients and families on the safe use of medications and potential adverse effects.

16. Stay informed on current trends and advancements in the field by engaging with literature, attending relevant seminars and conferences, and completing required continuing education credits.

17. Participate in departmental and hospital educational initiatives, including journal clubs, grand rounds, and professional organizations.
18. Contribute to the achievement of established goals and objectives while adhering to departmental policies, quality standards, safety protocols, and organizational values.

19. Comply with governmental and accreditation regulations.
20. Engage in meetings and committees, representing the department and the organization in community outreach efforts as appropriate.
21. Participate in multidisciplinary quality and service improvement teams.

Minimum Qualifications
Education:
Doctoral degree or equivalent Pharmacy degree from an accredited institution is required.

Experience:
1-2 years of direct patient care experience in a hospital environment is required. Completion of a PGY-1 pharmacy residency is preferred.

Licenses and Certifications:
PHARMD - Licensed Pharmacist in Washington, DC is required. Board certification is preferred.

Knowledge, Skills, and Abilities:
Strong verbal and written communication skills. Basic computer proficiency is preferred.
