Scheduler Coordinator/Case Manager

  • Schedule patient appointments accurately and efficiently, balancing provider availability and patient needs.  
  • Confirm patient appointments and provide reminders via phone, email, or HIPAA secured text messages.  
  • Complete eligibility check of insurances one day prior to visit  
  • Reschedule or cancel appointments as needed, ensuring timely communication with patients and providers.  
  • Address patient inquiries and concerns regarding appointments, wait times, and other scheduling-related issues.  
  • Collaborate with healthcare providers to manage their schedules, including accommodating special requests and adjusting for emergencies.  
  • Communicate any changes in the schedule to providers promptly.  
  • Maintain accurate and up-to-date patient records and scheduling data in Nova Medical Services electronic health record (EHR) system.  
  • Prepare and distribute daily, weekly, or monthly scheduling reports 
  • Monitor and manage appointment waitlists to fill last-minute cancellations and optimize provider schedules.  
  • Coordinate with other departments to ensure seamless patient care and service delivery.  
  • Other duties as assigned. 
  •  Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation.  
  •  Manage discharge plans upon completion of treatment 
  •  Work collaboratively with patients, families, physicians, and nurses to ensure high quality care  
  • Act as the patient's advocate as it relates to insurance coverage and financial assistance 
  •  Maintain the patient's comprehensive clinical record through detailed documentation 
  •  Coordinate with third party payors on a regular basis.  
  • Assist in processing laboratory orders, diagnostic tests, and more, ensuring adherence to protocols and patient safety 
  • Communicate patient needs and updates to healthcare team members, facilitating comprehensive care delivery. 
  • Advocate for patients in navigating insurance coverage and accessing financial assistance resources. 
  • Educate patients and their families on treatment plans, medications, and self-care techniques as needed. 


Qualifications:

  • 1-2 years of clinical and admin experience, preferable a certified medical assistant.
  • Proficient computer skills including experience with Microsoft Excel  
  • Strong written and verbal communication skills, must be bilingual in English and Spanish 
  • Excellent analytical skills High School Diploma or GED 
  • Excellent organizational and time management skills  
  • Familiarity with professional and technical emerging knowledge  
  • Problem solving skills and ability to multi-task  
  • Compassionate with teamwork skill 
  • Strong understanding of medical terminology, insurance procedures, and billing practices.  
  • Ability to maintain confidentiality and adhere to HIPAA regulations
