Job Summary and Responsibilities
The Case Manager (CM) is responsible for delivering high-quality, patient-focused care by overseeing rehabilitative services to achieve optimal patient outcomes. The CM coordinates and directs care tailored to the specific needs of each rehabilitation patient and collaborates with the Rehabilitation Program Director and Facility Case Management Director regarding departmental operations.
Key Responsibilities:
- Facilitate program orientation for patients, families, and caregivers, covering the role of the case manager, rehabilitation philosophy, and criteria for continued care and discharge.
- Act as the primary coordinator for patient and family education, encouraging their involvement in discussions regarding care plans, goals, and progress.
- Ensure the execution of the patient’s treatment plan that aligns with their strengths, needs, and preferences, promoting active participation throughout the rehabilitation journey.
- Document findings from the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating any social, financial, or discharge needs and preferences.
- Promote positive patient interactions that enhance the quality of service provided.
Qualifications:
- Registered Nurse with active state licensure; BSN preferred.
- A minimum of three years of clinical nursing experience in a hospital setting, with preference for candidates with two years of case management or critical care experience.
- Certification in case management or utilization review is preferred.
- Experience with InterQual is preferred.
