Key Responsibilities

  • Collaboration and Communication: Work closely with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care and effective communication.

Requirements
  • Licensure and Certification: Must be a Licensed Master's Social Worker (LMSW) with American Case Management Certification (ACM) or obtain certification when eligible.
  • Experience: Three years of acute hospital care or social work experience, with a strong background in case management and community resource coordination.
  • Skills and Abilities: Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
