For more information, please visit our website at www.opendoormedical.org.About the Role: Open Door is currently seeking a part-time Cancer Screening Clinical Consultant (Advanced Practice Clinician) with a potential full-time opportunity. This role is the clinical resource for CSP of the Hudson Valley staff, assisting the CSP Program Director with implementation of the CSP contractor deliverables as it relates to clinical services, case management, and quality assurance. Primary responsibilities include: interpreting clinical reports, guiding staff follow up and case management related to abnormal results, ensuring effective systems/processes in place between CSP Care Coordinators, CSP participating providers, and CSP enrolled clients to address timely/appropriate follow up for clients with abnormal cancer screening results, and assisting in interpretation of CSP policies and guideline concordant care for CSP participating providers.Responsibilities: Provides clinical oversight for all CSP staff for interpretation of reports and medical records; guidance for assessing risk, eligibility, and clinical appropriateness for screenings; and adherence to guideline concordant care. Interprets CSP policies and collaborates with the State Health Department as needed to ensure consistent/accurate interpretation of guidance and to provide information as requested; trains and updates staff and providers regarding CSP policies and procedures. Works with Care Coordinators to identify and establish effective and efficient processes with providers to ensure timely follow-up services. Assists Care Coordinators, participating providers, and patients to understand recommended follow-up of abnormal screening exams and treatment referrals. Provides clinical orientation and ongoing training of Care Coordinators and Intake Data Specialists. Provides training and ongoing support to participating and referring medical providers regarding CSP clinical guidelines and program overview. Provides support to Care Coordinators, participating providers, and community partners to help them understand the importance of and eligibility for preventive services. Works with CSP participating providers within the service area and other community resources to stay informed of service availability. Assists the Care Coordinators in determining what information is required and available to complete CSP data reporting forms and provides support on using the Catalyst data system to monitor clients in need of follow-up services. Conducts case reviews to ensure timely and correct follow-up, and reports metrics, challenges, and successes to program leadership. Builds awareness of care/data coordination and CSP services among the health care team by building professional relationships with participating providers and providing technical assistance and information about CSP. Assists Care Coordinators to identify resources and care plans for CSP eligible clients who are quot;at riskquot; with barriers to care (social determinants of health). Other duties as assigned. Qualifications: EDUCATION/EXPERIENCE: NYS Licensed Physician Assistant (PA) or Advanced Practice Nurse (NP). Background in primary care and/or women#x27;s health required and experience in cancer care and/or case management experience preferred. Must be knowledgeable about health care systems. Experience interpreting and following up on clinical reports and results. Must have a working knowledge of HIPAA regulations. Required Skills: Highly organized and self-directed, able to multi-task; flexible and exhibits the ability to adapt as situations evolve. Proficient computer use, including Microsoft Office applications. Effective in problem solving and exhibits excellent judgment and the ability to prioritize. Strong interpersonal skills including excellent active listening and clear (articulate) oral communication skills; engaging, warm, patient, and caring personality. Works effectively on teams. Strong writing and reading abilities; understands and follows detailed instructions. Strong client-centered orientation; collaborative; empathetic and driven to assist clients; passionate about providing high-quality care to low-income, under-served individuals and those lacking access to needed services.
