BEHAVIOR SUPPORT COORDINATOR:


$26-29/hr DOE

The Behavioral Support Coordinator provides behavior consultation, support, and coordination of mental health, addictions counseling and behavioral interventions. We are seeking an individual who has training on motivational interviewing and will work directly with Individuals to resolve conflicts that lead to behavioral concerns or that could jeopardize Individual safety. In addition, this individual will assist in screening all referrals, be part of the Service Planning Team and be available to provide on-call services.


Responsibilities include evaluating, developing, documenting, training, and providing services to address Individual behaviors, as well as development, coordination and implementation of counseling, education and treatment of substance use disorders for Individuals and their identified support network and ensure direct care staff training.


You will have the opportunity to provide services to a diverse group of adults with mental health and co-occurring medical and/or substance use conditions. This includes individuals with a variety of diagnoses or experiences including, but not limited to anxiety, depression, ADHD, PTSD, Bi-polar disorder, and psychosis.


Our team provides collaborative team based care, so you will work closely with case managers, psychiatry providers and RN’s for treatment planning and service coordination.

· Preparing concise and complete assessments, service plans and documentation related to client treatment, progress and services in a timely and professional manner.

  • Working effectively both independently and in a team setting.
  • Screening, assessing and diagnosing clients for services.

· Effective written and verbal communication.

  • Establishing and maintaining effective working relationships with other employees, residents and their families, other agencies, and the public at all times.
  • Acting professionally with regard to code of ethics and confidentiality.

What We Are Looking For:

  • A Bachelor’s degree occupational therapy, psychology, social work, or behavioral science.


Special Qualifications:

Education and experience which demonstrates the competencies to identify precipitating events; gather histories of mental and physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts; assess family, social and work relationships; conduct a mental status examination; document a multiaxial DSM diagnosis; write and supervise a treatment plan, conduct comprehensive mental health assessment; and provide individual, family, and/or group therapy within the scope of practice.
