Multnomah County Health Department's Behavioral Health Division is seeking social workers and mental health professionals who represent the communities we serve and are dedicated to removing barriers to youth and families who experience intersectionality to join our team as the Clinical Services Specialist - Intensive Care Coordinator. As the Intensive Care Coordinator you will play a crucial role in screening, analyzing, assessing, care planning, and coordinating ongoing care for our members. You will help navigate the behavioral health system, facilitate collaboration among care team providers, and support transitions of care to achieve optimal health outcomes.
Clinical Services Specialist - $1,500 Hiring Incentive
**As a newly hired Clinical Services Specialist, you will receive a one-time New Hire Recruitment and Retention Incentive payment of $1,500 on your first paycheck. The incentive will be prorated based on the position's FTE status**
Teleworking/Hybrid: At this time, this position is designated to "routine telework/hybrid," meaning you will be working remotely (from home) most of the time and you will be required to attend meetings in the community. As with all telework positions, employees may be required to come into the office on occasion. The designation of Routine Telework may be subject to change at a future time. Currently, all employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations.
As the Clinical Services Specialist - Intensive Care Coordinator duties include:
Intensive Care Coordination
- Conduct advanced clinical assessments, analyses, and treatment services involving complex psycho-social factors.
- Administer standardized behavioral health assessments and interventions for clients with complex needs.
- Develop and integrate comprehensive care plans in collaboration with Care Coordination Services team members, clients, their support systems, community partners, and clinical care providers.
- Provide outreach, engagement, and supportive services to ensure access to necessary care and services.
- Utilize appropriate assessment tools for neuropsychological, psychological, psychiatric, psycho-diagnostic, or clinical case consultations.
- Build and maintain working relationships with clients, their support systems, and external partners.
- Assist members in accessing social services programs, including substance treatment, housing, peer support, and medical services.
- Ensure strength-based care plans that reflect racial, cultural, and ethnic factors.
- Evaluate referrals to determine the need and appropriateness for Intensive Care Coordination.
- Provide consultations and clinical evaluations to community partners and multi-disciplinary teams.
- Offer guidance to community members seeking assistance for friends or family.
- Communicate with regional coordinated care organization (CCO) partners for referrals to lower levels of care.
- Maintain timely and accurate documentation, including Releases of Information (ROI) requests and service plans.
- Complete notes for billing and grant funding purposes.
- Enter documentation into tracking systems or electronic medical records to record outcomes.
- Report findings from investigations, urgent or specialized needs, and potential policy violations.
- Participate in ICC team meetings, huddles, and training sessions.
- Build and maintain strategic relationships with community partners and providers.
- Share updates with program and division leadership to tailor program goals and identify community needs.
- Attend community outreach meetings and act as a contact for multi-disciplinary teams.
- Respond to emergency needs or investigation requests involving clients or service providers.

Required Minimum Qualifications/ Transferable Skills*:
Education/Experience:
- Equivalent to an Associates degree in social services, psychology, sociology, counseling or related field and four (4) years professional work experience as a counselor, advocate, or social worker ; OR,
- Equivalent to a Bachelor's degreein counseling, social services, psychology, sociology, social work or a related field andtwo (2) years of professional work experience as a counselor, Mental Health treatment professional, social worker, or therapist; AND
- Must meet the standards of Qualified Mental Health Associate - (QMHA-R) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at MHACBO website. Current QMHA-R (Required)
- Valid Driver's License
- This position requires a Background Investigation, which may include being fingerprinted.
- Working knowledge of Google Suite, EvolvNX and Epic databases preferred.
- Experience working within the adult system of care including crisis and acute care settings, shelter and housing, and mental health services. .
- Experience working with individuals who have Oregon Health Plan, Health Share and CareOregon
