Homelessness remains a critical issue affecting numerous individuals in California. Illumination Foundation (IF) is committed to breaking the cycle of homelessness by delivering specialized services through a network of facilities spread across Orange County, Los Angeles County and the Inland Empire.

Overview

  • Utilize diverse communication tools and strategies to engage with members
  • Implement culturally sensitive approaches to outreach and enrollment
  • Manage documentation and engagement frequency effectively
  • Utilize different communication modalities for effective outreach
  • Employ various techniques including trauma-informed care and motivational interviewing
  • Responsibility to enroll a percentage of assigned caseload

Responsibilities

  • Effectively communicate with members, care team, and other relevant parties
  • Document all engagements accurately and promptly in designated platforms
  • Utilize communication modalities tailored to individual member needs
  • Demonstrate proficiency in time management and communication efficiency
  • Implement specialized techniques like trauma-informed care and de-escalation
  • Enroll a specific percentage of assigned caseload

Requirements

  • High School Diploma or equivalent
  • Minimum of 2 years related experience
  • Basic computer skills, including proficiency in Microsoft Office Suite
  • Valid CA Driver's License and eligibility for company vehicle insurance
  • Preferred bilingual proficiency in English and Spanish
  • Knowledge of Medi-Cal Managed Care Plan systems
