Ecumen Hospice is currently seeking a Registered Nurse (RN) Case Manager to cover the South Metro area, including Bloomington, Richfield, Apple Valley, Lakeville, and Burnsville. Our mission at Ecumen Hospice is to provide exceptional care and support to individuals in the later stages of life, ensuring comfort and quality of life.
The RN Case Manager will be responsible for delivering direct care and case management services to a caseload of clients following agency standards. They will provide skilled nursing services in the home and community, meeting the nursing needs of patients, families, and groups of all ages.

Key Responsibilities:
- Deliver efficient and effective skilled services to meet the needs of patients and families.
- Conduct thorough assessments of patients and families to identify hospice requirements, including physical assessments and medical history.
- Collaborate with healthcare professionals, families, and clients to develop comprehensive care plans.
- Create objective care plans for patient health and functional improvement, regularly evaluating and updating as needed.
- Ensure accurate communication of physician orders to agency staff.
- Initiate case conferences to address team responsibilities, client progress, and ongoing care plans.
- Promote a safe environment for clients and colleagues.
- Other duties as assigned.
