About the Position:
We are on the lookout for a skilled and dedicated Clinical Liaison to enhance the effectiveness of the Home Health clinical team through continuous training, coaching, and development. Reporting to the VP of Home Health, you will be responsible for creating, delivering, and enhancing the orientation and clinical education program, as well as providing ongoing skills development training for the team.
- Provide personalized clinical training to all field RNs and LPNs during initial home visits
- Deliver direct patient care in their homes, including OASIS, and assess clinical skills competency
- Conduct virtual and in-person clinical education sessions, ensuring compliance with quality standards
- Orient new clinicians to the care center(s), including policies, procedures, documentation, compliance, and regulatory requirements
- Facilitate learning experiences for mentees, promoting the acquisition of new knowledge and skills through mentoring and coaching
- Evaluate mentees' capabilities and provide feedback for improvement
Qualifications:
- Bachelor's degree or higher in Nursing & RN License
- Several years of experience in Certified Homecare
- OASIS Proficiency
- Expertise in various clinical skills such as wound care, infusions, and catheter management
- Familiarity with EMR systems including Epic, McKesson, HomeCare HomeBase (HCHB)
- Experience in virtual education programming, clinical supervision, and training
