Fir Lane Health & Rehabilitation Center is part of the Caldera Care family. At Caldera Care we're known for our commitment to our staff and local communities in which we serve. Our goal is to provide the best possible care for our residents and instill a sense of confidence within the community. When a loved one comes to our skilled nursing facility, they will be well cared for, will feel heard, and will know that they matter. We strongly believe that the key to this is through our incredible staff

What you will do:

  • The Social Services Assistant assists the Social Services Director in providing services to meet the social and emotional needs of the residents/patients.
  • Develops a rapport with the residents/patients and presents their concerns to center management.
  • Charts as assigned
  • Assists with Resident Council
  • Assists the Social Services Director with discharge planning for residents
  • Follows all center's policies and procedures

Qualifications:

  • High school diploma or equivalent
  • Ability to speak, understand, write and read in English
  • Previous experience in a long term care setting PREFERRED
