Job Summary
Sts. Joachim and Ann Care Services is seeking a Housing Intake Specialist who will assist with addressing immediate, intermediate and long terms needs of individuals and families who are homeless or receiving homeless assistance. This position is responsible for interviewing, evaluating, and gathering supporting documents to prove qualification for specific grants/assistance provided through the agency.  This includes recording ongoing case activities, communicating with recipients to ensure all needs are addressed, and referral to other agencies where appropriate.  Hourly rate is $15.45; Full-time; 40 hours per week.

Job Responsibilities
-Act as the Service Advocate for homeless or in crisis families, by gathering information and documents from recipients to assess their needs.
-Effectively facilitate certifications and assessment process across the three counties
-Determine eligibility and best available assistance in consultation with the Housing Program Supervisor.
-Under the direction of the Housing Program Supervisor, assist with calling in pledges to vendors or putting in the pledges in on online portals, then calling the clients to alert them of pledges.
-Assist with overseeing volunteer intake workers by providing support, resources, answering recipient assistance questions, and organizing the daily certifications.
-Refer families to specialized agencies, including making calls to other agencies as needed, for any complex needs that cannot be handled by the Care Service.
-Contact landlord, utilities as needed to confirm assistance.
-Follow through to confirm client assistance is completed and file has required forms and documentation for assistance.
-Maintain documentation client information including immediately & accurately entering case notes, service activities, and outcomes into the system within 48 hours of contact.
-Monitor services to ensure that contracted and pledged services are performed according to agency and grant funding standards. 
-Maintain confidentiality for all personal recipient information and assure that program participant’s rights are protected.
-Maintain understanding of grant guidelines and housing/homelessness best practices.
-Use computer systems and information in them on a NEED-TO-KNOW basis -only for looking at cases you have been assigned or must directly work on. 
-Assist in entering data and maintaining accuracy in client files and electronic databases (Bell Data and CISS - Caseworthy).
-General office tasks; work processing, copying, filing, faxing, mail processing, shredding, organizing, file maintenance.

Job Requirements
-Communicate effectively both verbally and in writing with people of various socio-economic and cultural backgrounds including communication with: Director of Program Services, administration, recipients, volunteers, community partners, funding agency representatives, intake workers, etc.
-Ability to keep confidential information and only share client information on a need-to-know basis required for the job. 
-Have sharp problem-solving skills and be able to make decisions quickly in very stressful situations.
-Experience with and ability to use computer (Word, Excel, and Bell Data), fax/copier and telephone.
-Exercise good judgment and use good organizational skills in evaluation situations, making referral decisions and following through.
-Knowledge of Community and agency resources.
-Knowledge of principles and methods of interviewing.
-Skills in case documentation.
-Work harmoniously with applicants, recipients, public and other employees.
-Work in and promote a team environment

General-Work Related Experience:

-Ability to work in highly detail- and task-oriented, fast-paced environment
-1-3 years’ experience in social service setting
-1-2 years’ experience in intake and case management within social service setting preferred, but not required
-Experience working with vulnerable populations (homeless, substance abuse, mental health, etc.), preferred, but not required

Education:

-High school education or GED (required)
-Bachelor’s degree in Human Service field preferred, but not required
