ROLE RESPONSIBILITIES AND ACCOUNTABILITIES BUT ARE NOT LIMITED TO THE FOLLOWING:
Collaborates with primary care providers at Eastern Kansas VAMC to provide nursing care to Veterans in their home.
Tracks and coordinates ongoing patient care, collect and report program evaluation data as assigned by leadership.
Documents outcomes for care and services provided at the program level.
Provides education, coordinate patient care, and collect/report evaluation data as directed.
Uses clinical and collaborative expertise and interpersonal communication to educate and guide interdisciplinary teams in the appropriate application per the VA Handbook.
Other duties assigned.
