Schedule : Monday-Friday 7:30 am-4:00 pm
Summary of Job Duties : This position is responsible for educating the public, community partners in care, and referral sources about the Community LIFE program, organizing community outreach and promotional activities, and guiding interested elders into the intake process. This position involves local travel up to 50% of the time (mileage reimbursed).
Bachelor’s degree preferred in Business, Communications, Marketing or Health related field.
Knowledge of physician, hospital, skilled nursing facility, case management and discharge planning service needs.
Ability to form positive interpersonal relations in dealing with a wide range of staff as well as external contacts
Ability to multi task, be organized, and manage time appropriately
Computer literacy including ability to use Microsoft Word, Excel, and Outlook.
Ability to promote the program in a positive, patient approach that builds trust in the Community LIFE brand and contributes to continued growth and expansion for the company
Community LIFE is a program of all-inclusive care for the elderly, committed to empowering older adults to remain at home while preserving their dignity, independence and quality of life.
Our program brings the region’s experts in geriatric medicine and care together to work as a team in specialized Day Centers, to help older adults enjoy the highest quality of life possible.
Our professionals are committed to keeping older adults independent, and in their homes. Our wide range of services are designed to meet the varied needs of seniors, and include medical care, social services, meals, activities, transportation and much more.
