We're looking for an outstanding Lead Care Manager to join our field-based team in Santa Clara, CA. Reporting to the Director of Enhanced Care Management, ECM Clinical Manager, and/or ECM Program Manager, the Lead Care Manager (LCM) assumes the crucial role of being the primary point of contact for clients. In this position, the LCM works closely with various providers including doctors, specialists, pharmacists, social services providers, and others to ensure alignment regarding the client’s needs and care. 
Responsibilities include managing client cases, coordinating healthcare benefits, offering education, and facilitating timely and cost-effective access to care. Additionally, the LCM collaborates and communicates with the client’s caregivers/family support persons, other providers, and members of the Care Team to promote wellness, recovery, independence, resilience, and member empowerment. In-Person care management duties as described below: 
Assess member needs in the areas of physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referral and linkage to community-based services and supports 
Oversees the development of the client care plans and goal settings 
Offer services where the member resides, seeks care, or finds most easily accessible, including field-based or telehealth 
Advocate on behalf of the client with health care professionals (e.g. Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles 
Conduct outreach and engagement activities in order to facilitate linkage to the ECM program and log activity in the Client Relationship Management (CRM) system
Complete all documentation, including outcome measures within the timeframes established by the individual care plans 
Maintain up-to-date patient health records in the Electronic Medical Record (EMR) system and other business systems 
Attend training as assigned 
Willing and able to work Monday-Friday 8:30am-5:00pm, 70- 80% of the time in the field and 20- 30% remotely.
~2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations 
~ Working knowledge of government and community resources related to social determinants of health
~ Must have general computer skills and a working knowledge of Google Workspace, MS Office and the internet

The hourly rate for this position will be based on several factors, including, but not limited to: your prior experience and skills related to the position, geographic location, company needs, and current market demands. 
Mileage reimbursement per IRS guidelines.
Including 401(k) with match, health, dental, & vision insurance, STD/LTD insurance, and paid time off.
Works at all times within the policies and guidelines of the company
Ability to use video conferencing frequently and consistently
Respects at all times the confidentiality of information covering patients, staff, and volunteers and is HIPAA compliant
