Details:

As the Registered Nurse (RN) Field Admissions Specialist at Suncrest Hospice, you will complete hospice admissions and patient visits as needed. Admission nurses are responsible for orienting new patients to the hospice program, assisting the patient and caregiver with all legal paperwork, and completing a thorough nursing assessment.

Qualifications:
Good understanding of hospice rules and regulations
Ability to establish both hospice eligibility and diagnosis when patient is appropriate
Ability to manage time efficiently to balance visits, documentation and meetings
Willingness to travel to meet our patients where they live (private homes, assisted living facilities, hospitals, etc.)
Reliable transportation, valid driver’s license, and auto insurance
Registered Nursing Licensure in the state of California
