Responsibilities

As the Discharge Planner , you'll play a critical role in facilitating timely and effective discharges for our patients. Under the supervision of our Director of Social Services, you'll work closely with our interdisciplinary team to ensure a seamless transition from our facility to the next level of care.

Your Impact:

Help patients and families navigate the discharge process and ensure a smooth transition Collaborate with our interdisciplinary team to provide comprehensive care and support Utilize our electronic medical records (EMR) system to track patient progress, document case management notes, and support individualized treatment plans Foster open communication lines with our Director of Social Services, Program Managers, and other staff members to ensure seamless care coordination

Qualifications

What You'll Bring:

Master's degree in a mental health field from an accredited college or university A passion for working with patients and families and a commitment to delivering high-quality social work services Experience with discharge planning, case management, and patient advocacy Strong communication, interpersonal, and organizational skills Ability to work collaboratively as part of a multidisciplinary team Knowledge of external review organizations, payor sources, financial planning, and contracting (e.g. managed care, Medicare, Medicaid, Tricare)
