Position Overview:

  • Exhibits leadership capabilities by designing and implementing innovative programs that align with the Facility's overall mission.
  • Handles inquiries from potential residents and keeps detailed records of these inquiries.
  • Conducts interviews with patients or their family members during the admission application process.
  • Clarifies the financial aspects of care associated with the recommended level of service as advised by the admitting physician.
  • Provides necessary documentation to gather comprehensive medical and social information for review.
  • Acts as a liaison among families, hospital discharge planners, social service organizations, and medical professionals.
  • Upon availability of an appropriate bed and completion of all records, communicates the admission time in accordance with Facility policies.
  • Informs families about admission approvals, including room assignments and expected arrival times.
  • Alerts all relevant departments regarding new admissions.
  • Welcomes patients and their families upon arrival, assisting them in settling into their rooms as needed.
  • Completes the cover sheet of the MDS form.
  • Leads the Admission Conference/MDS meeting for new residents.
  • Facilitates patient-family conferences as necessary.
  • Participates actively in required meetings.
  • Assists families or residents with discharge planning and provides necessary discharge information while maintaining a list of community resources.
  • Addresses complaints from residents or families, investigates issues, and reports findings to the Department Director.
  • Processes change of room requests and notifies relevant departments accordingly.
  • Refers residents or families to appropriate agencies or resources when services cannot be provided within the Facility.
  • Collaborates closely with Unit Directors/RN Supervisors, administrative nursing staff, and other Social Workers (if applicable) regarding admissions and room changes.
  • Drafts correspondence as required, adhering to established procedures.
  • Monitors completion of necessary sections on the MDS within 14 days of patient admission, utilizing Quality Assurance measures.
  • Ensures that Social Work progress notes and electronic assessments are completed in accordance with documentation policies.
  • Guarantees that Social Workers meet residents' social needs through scheduled visits.
  • Supports Family Council activities by providing discussion topics and offering assistance as needed. May be required to lead resident council meetings.
  • Receives notifications regarding residents with precautionary conditions and follows Facility protocols.
  • Engages in Quality Assurance committee activities by performing monitoring tasks and reporting to the Q.A. Committee.
  • Meets monthly with Social Workers (if applicable) to address concerns and suggestions for departmental improvement, reporting findings to the Department Director.
  • Encourages a professional atmosphere within the department while maintaining focus on Facility objectives.
  • Assists in delivering annual training sessions for staff.
  • In emergency situations, such as fire drills:
    • Shut off all electrical equipment.
    • Close doors.
    • Report to the Nurses Station for further instructions.
  • Performs additional duties as assigned.

Qualifications:

  • High school diploma required. One year of experience in healthcare or long-term care preferred, or an equivalent combination of education and experience.
  • Ability to work collaboratively with colleagues.
  • Strong problem-solving skills.
  • Ability to perform well under pressure.
  • Maintains a neat appearance and good personal hygiene.
  • Adheres to appropriate business attire.
  • Familiarity with personnel, departmental, and Facility policies is essential.
  • Knowledge of local, state, and federal regulations related to patients' social needs is required.
  • May be required to run errands as necessary.
  • Occasionally assists with transporting patients to medical appointments, which may involve driving or accompanying patients.
  • A valid driver's license is required.
