Overview

Under the supervision of the Community Habilitation Supervisor, the Community Habilitation Specialist (CHS) is responsible for assisting individuals to become more independent when performing activities of daily living and community integration in order to achieve their valued outcomes. Habilitation services are designed to develop skills in order to increase an individual’s level of independence. The Habilitation services that the Community Habilitation Specialist is responsible for organizing and implementing includes but is not limited to: training, supervision, or assistance related to communication skills, interpersonal skills, socialization, mobility, community transportation/travel training skills, problem solving skills, money management, life safety skills, etc. Services are primarily community based and revolve around volunteer and recreational opportunities. The Community Habilitation Specialist is responsible for fostering and maintaining an environment that promotes the health and safety of all persons served and must abide all agency policies, procedures, mission, vision, etc.

Responsibilities

Essential Job Functions

Provides direct services that support the participant(s) in a least restrictive manner. The CHS will teach, assist, and motivate the participant(s) in identifying activities that reflect the participant(s) wishes based on the participants objectives and valued outcomes identified in the Individual Service Plan (ISP) and assist them with developing more independence in those areas. Responsible for ensuring the supervision of participant(s) based on individuals abilities including areas of self-care, independent living, community integration, etc. Completing documentation of services provided in accordance with regulations, policies, practices and procedures. Respond appropriately to crisis/emergency situations in order to ensure and secure the safety and well-being of participant(s). Response includes acting appropriately to severity of the situation, adherence to regulatory/agency guidelines, communication and documentation of situation to appropriate staff, honor the participant’s right to assume risk in an informed manner, etc. Prevent abuse and correctly follow procedures for responding and mandated reporting to Agency and Justice Center representatives/personnel as directed. Work with participant(s) and their support network to identify areas of risk and to create safeguards to these concerns. Proactively and respectfully address challenging behaviors with the participant & their Supervisor as needed. Demonstrate understanding and practice of SCIP-R techniques (this will be on a case by case basis and determined prior to working with the participant). Demonstrate professionalism, leadership, and diversity. Provide quality service/care with respect and dignity for participant(s).  Demonstrate professionalism, respect and ability to work in a team environment. Maintain confidentiality Report to work on time and maintains regular attendance in accordance with Agency policies and practices. Complete and maintain all required agency trainings in accordance with the Agency policies and practices. This includes: completing required agency trainings (specifically monthly required Relias courses & any other courses the Community habilitation Supervisors assign to staff per quarter), First Aide & CPR. SCIP-R will be required on a case by case basis. It will be determined if it is needed by the Community Habilitation Management team based on the case. However, if the employee has a dual role and works in another FREE location that requires SCIP-R, then the employee must take SCIP-R upon hire and maintain their certification each year. This will be monitored and tracked by the Community Habilitation Supervisors. Attend any division or team meetings as well as any supervision meetings with the Community Habilitation Management team Attend your cases IDT-Interdisciplinary team meetings/team/ISP-Individualized Service plan meetings as well as any required behavioral trainings for the individual or Community Habilitation program

Perform other duties as assigned

Qualifications

Minimum Job Qualifications

18 years of age or older High School Diploma or equivalent required plus one year experience working with Developmentally/Intellectually Disabled individuals preferred Valid driver’s license that meets all the requirements to be a valid driver for the agency. Ability to be flexible and work in a team environment Excellent interpersonal, oral and written communication, decision-making, and problem-solving skills a must Computer skills and keyboarding familiarity required Must be able to be reached by phone and have a vehicle to transport and complete agency business

USD $ 20.50/Hr - $22.73/Hr
