The Atrium at Navesink Harbor is seeking an experienced Director of Social Services to join our team

The Atrium at Navesink Harbor is a Springpoint Senior Living Community located in Red Bank, NJ. At Springpoint, the Director of Social Services is responsible for assessing individual psychosocial and emotional needs of residents through the interview process within fourteen (14) days of admission and to develop individual plans of care with interdisciplinary care plan team members with a focus on desires of resident.

Responsibilities:

Meet with new residents within three (3) business days to discuss availability of Advance Directive, answer questions, provide Health Care Center Resident Handbook and Resident Bill of Rights.Complete initial Social Service Assessment and Social History within fourteen days. Complete written assessment of Memory Care residents upon admission; update every six months thereafter.Update and reassess Healthcare residents every three months.Plan, implement, and provide services to residents in concert with other disciplines while responding to the needs of such residents.Coordinate interdisciplinary resident care plan meetings through preparation of weekly schedule. Send invitations to family members to annual review of care plan or when a change in status necessitates the completion of a new MOS and subsequent care plan conference. Seek to involve family members in the life of the facility through written invitations to resident care plan meetings, Family Council meetings, telephone calls when a need for contact is identified.In concert with Chaplain, organize and meet with Family Council in Heath Care Center, Assisted Living, or Memory Unit with participation of other disciplines to present relevant programs, discuss matters of concern, and educate families.In-service staff in Health Care Center, Assisted Living, and Memory Unit on abuse prevention, detection, and reporting annually.Develop and facilitate ongoing staff education in areas related to social service including such original programs as "Psychosocial Ethics."Receive training in relevant areas to increase knowledge applicable to service provision to residents, obtain specific skills, and earn continuing education credits for clinical social work licensure.Perform role of advocate for resident as needed.Integrate the social service department with other services in the facility to provide a continuum of care.

Education and Experience:

Bachelor's Degree requiredCSW or LSW required3+ Years experience with Seniors Experience in Long Term Care
