Primary Responsibilities: Provide physical therapy services to patient according to physician’s plan of care. This may include, but will not be limited to assessing and evaluating therapeutic / rehabilitative / functional status, participation in the development of the total plan of care and evaluates home environmentInitiates physical therapy program and instructs other personnel and / or family caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferencesPrepares and submits clinical and progress summaries in a timely mannerParticipates in discharge planning for patientSupervises physical therapy assistants according to organization policy and state regulations, treat and re-evaluation patient no less than every 7th day of treatment or 21 days whichever occurs firstMaintains contact / communication with other personnel involved in the patient’s care to promote coordinated, efficient careMaintain clinical competency in physical therapy practice and theoryWill participate in Agency on-call coverage as outlined in Agency policy You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications: Graduate Degree from an Accredited Physical Therapy ProgramActive and unrestricted Nevada Physical Therapist licenseCPR certification or obtain within 30 days of hire2+ years of experience as a Physical TherapistAccess to reliable transportation that will enable you to travel to client and/or patient sites within a designated area Preferred Qualifications: 1+ years of Home Health experienceExperience with Electronic Medical Records SystemsExperience with OASIS documentationExperience in a home health settingWeekend availability Nevada Residents Only: The salary range for this role is $70, to $, annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
