We are seeking a skilled Physical Therapist to join our Home Health team in Las Vegas, NV. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the success of our team.
Key Responsibilities:- Provide physical therapy services to patients according to physician's plan of care, including assessing and evaluating therapeutic/rehabilitative/function status.
- Initiate physical therapy programs and instruct other personnel and/or family caregiver members in certain phases of physical therapy.
- Prepare and submit clinical and progress summaries in a timely manner.
- Participate in discharge planning for patients and maintain contact/communication with other personnel involved in patient care.
- Supervise physical therapy assistants according to organization policy and state regulations.
- Graduate degree from an accredited physical therapy program.
- Active and unrestricted Nevada Physical Therapist license.
- CPR certification or obtain within 30 days of hire.
- 2+ years of experience as a Physical Therapist.
- Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area.
- 1+ years of Home Health experience.
- Experience with Electronic Medical Records Systems.
- Experience with OASIS documentation.
- Experience in a home health setting.
- Weekend availability.
