Responsibilities:

- Conduct assessments of client, needs, strengths, and goals.
- Develop individualized service plans in collaboration with clients, ensuring that their unique
needs and preferences are prioritized.
- Utilize personal vehicle for client visits, transportation to appointments, and other work-related
travel.
- Provide ongoing case management services, including regular follow-ups, monitoring
progress, and adjusting service plans as needed.
- Connect clients with appropriate community resources, services, and support networks to
address their needs and promote self-sufficiency.
- Maintain detailed and accurate documentation of all client interactions, assessments, service
plans, and progress notes in compliance with organizational and regulatory standards.
- Collaborate with other service providers, healthcare professionals, and community
organizations to coordinate comprehensive care for clients.
- Utilize data-driven approaches to track client outcomes, identify trends, and improve service
delivery.
- Advocate for clients' rights and needs within the community and service systems.
- Participate in ongoing professional development and training to stay current with best practices
and emerging trends in case management.

Preferred Qualifications:

- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field. Master’s
degree preferred.
- Minimum of 1 years of experience in case management, social work, or a related field.
- *Valid driver's license and reliable personal vehicle for work-related travel.
- Strong knowledge of community resources and social services.
- Excellent organizational, communication, and interpersonal skills.
- Proficiency in using data management systems and software for documentation and reporting.
- Ability to work independently and as part of a team in a fast-paced environment.
- Demonstrated commitment to consumer-centric and data-driven approaches in service
delivery.
- Passion for making a positive impact in the community and supporting individuals in achieving
their goals.
