Job Description

:Patient Navigator assists patients in navigating the health care system.

The hours for this position are 8:am-4:30 Mon- Fri

This can be a hybrid /flex position ( after the 6 month training period) 2 days in office and 3 days remote per week

Answers incoming calls from patients and providers. Triages calls and schedules patient office visits and testing appointments. Coordinates with clinical and support departments on schedule modifications. Requires critical thinking skills, decisive judgment, ability to collaborate with multiple teams and the ability to work with minimal supervision. Must be able to follow protocols and work in a fast-paced environment supporting multiple clinics and physicians.

Handles and directs calls that relate to pharmacy refills, surgery clearance, testing instructions, MyChart access, lab orders, Lyft scheduling, and interpreter assistance. Triages immediate patient concerns and collaborates with RN Navigator and Clinic RNs when appropriate. Schedules appointments for office and virtual visits. Complex scheduling of procedural and diagnostic testing appointments for multiple clinics and providers based on diagnosis and provider/clinic preferences; must be able to follow pre-determined protocols for scheduling and provide pretest instructions to the patient.

In addition to General Cardiology, the Patient Navigator schedules for subspecialties, Electrophysiology, Heart Failure, Vascular and Device studies. Documents all conversations, demographics to EMR timely and accurately. Obtains complete and accurate clinical, demographic, financial information during the scheduling process. Generates cost estimates for services as it relates to office visits and in-office testing; directs patients to Financial Counseling resources when appropriate. Follows guidelines for special requests/Urgent/schedule modification procedures and coordinates communications with clinical areas to accommodate the customers’ requests. Promotes mission, vision, and values of SCL Health, and abides by service behavior standards. Meets performance standards and call metrics as outlined by the department management. Performs other duties as assigned.

Minimum Qualifications

One year customer service.

Ability to collaborate with multiple teams.

Preferred Qualifications

One year in related/medical field.

Understanding of medical terminology.

Previous call center experience or CNA, MA or EMT experience.

Experience with Epic.

Bilingual.

Physical Requirements:

Interact with others by effectively communicating, both orally and in writing.- and -Operate computers and other office equipment requiring the ability to move fingers and hands.- and -See and read computer monitors and documents.- and -Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.- and -May require lifting and transporting objects and office supplies, bending, kneeling and reaching.
