This position requires a Bachelor's degree in a Human Service field (Psychology, Social Work, Sociology, or closely related study). A background check and valid driver's license are also required. As a Targeted Service Coordinator, you will provide case management for adults diagnosed with a developmental disability who receive Medicaid and non-Medicaid supports and services. You will be an advocate for clients on your caseload, conduct in-person and virtual client visits, and attend team meetings with collaborative stakeholders in the community (virtual and in-person). You will assist your clients with obtaining and maintaining services, as well as oversee progress and satisfaction with their chosen providers. This is not a direct care position and you will not provide any transportation or direct care.

This is a fast-faced job and requires an individual who enjoys being part of a team, helping others, and making a difference. The ideal candidate will be self-motivated, have good interpersonal and advocacy skills, and be willing to learn. Experience is not required, but it is preferred. New graduates are welcome to apply This job offers tremendous flexibility. Rate of pay depends on experience, but generally ranges from $32-$38 per hour.
