Better Me 2 Wellness is a Nurse Practitioner–owned and operated healthcare practice
offering mobile, in-home, and concierge medical services. Our mission is to provide
compassionate, patient-centered care that is accessible, convenient, and effective. We
serve diverse communities throughout Chicago and surrounding areas with services
ranging from primary care and chronic disease management to wellness, IV hydration,
and weight management.
Position Summary
We are seeking a dedicated Nurse Practitioner (NP) to join our growing team. The NP
will provide in-home medical care for patients primarily located on the West Side,
South Side, and nearby Chicago communities. This role is ideal for an independent,
compassionate provider who is passionate about bringing quality healthcare directly to
patients where they live.
Responsibilities
Provide comprehensive in-home medical evaluations, assessments, and follow-
up care.
Manage chronic diseases (hypertension, diabetes, COPD, etc.) and acute health
concerns.
Perform physical exams, order and interpret labs/diagnostic testing, and
prescribe medications as appropriate.
Develop and manage individualized treatment plans in collaboration with patients
and caregivers.
Document visits in EMR (Tebra experience preferred).
Educate patients and families on disease prevention, medication adherence, and
wellness strategies.
Coordinate care with specialists, home health agencies, and other providers as
needed.
Ensure compliance with HIPAA and other regulatory requirements.
Qualifications
Current Illinois Nurse Practitioner license with full practice authority (FPA
preferred).
Active DEA license (or eligibility to obtain).
Minimum of 1–2 years NP experience in primary care, internal medicine, family
medicine, or geriatrics.
Strong clinical decision-making and independent practice skills.
Excellent communication, organizational, and time-management abilities.
Reliable transportation and willingness to travel within assigned Chicago service
areas.
Preferred Attributes
Experience with home visits, house calls, or mobile care.
Comfort working with underserved and diverse populations.
Passion for patient-centered, holistic healthcare.
Knowledge of billing and documentation requirements for Medicare/Medicaid.
