The Clinical Leadership Associate will supervise and train primary care providers (PCPs) in assigned centers. You will play a pivotal role in enhancing clinical quality, improving patient outcomes, and ensuring clinician-patient satisfaction. In addition to your leadership responsibilities, you will engage in direct patient care and other clinical duties as needed.

Key Responsibilities:

  • Provide comprehensive care to patients with both acute and chronic illnesses, focusing on the needs of older adults.
  • Take accountability for patient care outcomes, seeking specialist consultations when necessary.
  • Collaborate with hospitalists for patients admitted to hospitals, ensuring seamless care management.
  • Assess, diagnose, treat, and manage patients with complex health needs while educating and promoting health.
  • Lead your care team, including medical assistants and coordinators, to provide effective patient care.
  • Engage with transitional care teams for patients unable to visit the office, coordinating care as needed.
  • Facilitate weekly Super Huddles and Transforming Care Meetings, supporting center leadership in their absence.
  • Actively participate in center management duties and cover for absent providers.
  • Mentor and coach PCPs to achieve exceptional clinical outcomes.
  • Partner with Center Operations Directors to drive performance improvements and expand center membership.

Knowledge, Skills, and Abilities:

  • Proficient in Microsoft Office Suite and other applications used by the company.
  • Willingness to travel locally for market needs and occasionally nationally for training or recruiting.
  • Fluency in English, with additional language skills beneficial.
  • Demonstrated ability to exercise independent judgment in clinical settings.

Education and Experience Requirements:

  • MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or a related specialty required.
  • Active MD licensure in the state of employment is essential.
  • A minimum of 1-year experience in a clinical setting serving older adults is preferred, ideally as a ChenMed PCP Partner.
  • Completion of Chen Medical training is preferred.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics is preferred; Board Eligibility is required.
  • DEA number for controlled substances is mandatory.
  • Basic Life Support (BLS) certification is required within the first 90 days of employment.
