As the Clinical Leadership Director, you will play a vital role in supervising and training primary care providers (PCPs) at your designated center. Your primary objective will be to enhance clinical quality, improve outcomes, and ensure satisfaction among both clinicians and patients. While you will keep a portion of your time for direct patient care, your leadership responsibilities will also significantly shape the clinical-operations synergy and culture of our team.
Key Responsibilities:
- Provide high-quality care for older adult patients managing acute and chronic conditions.
- Take full accountability for patient outcomes, engaging specialists for consultation as needed.
- Interact with hospitalists for admitted patients to ensure continuity of care.
- Oversee assessment, diagnosis, treatment, health promotion, and care management.
- Lead and coordinate a care team including care promoters, facilitators, and coordinators.
- Engage with transitional care teams for patients unable to visit the office.
- Facilitate weekly Super Huddles and Transforming Care Meetings.
- Participate actively in center management, filling in for absent providers and assisting in recruitment efforts.
- Mentor and coach PCPs to achieve outstanding clinical outcomes.
- Collaborate with Center Operations Director to enhance financial performance and membership growth.
- Perform additional duties as assigned.
Required Skills and Qualifications:
- Proficient in Microsoft Office Suite and related applications.
- Willingness to travel locally and nationally for training and recruitment purposes.
- Fluent in English, with other language skills a plus.
- Exercise independent judgment in decision-making.
Education and Experience:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related specialty is required.
- Current, active medical license in the state of employment.
- A minimum of one year of clinical experience in geriatrics, adult, or family practice is preferred.
- Completion of ChenMed training as part of an individualized development plan.
- Board certification in relevant specialty is preferred; Board eligibility is required.
- Must maintain current DEA registration for controlled substances.
- BLS certification from the American Heart Association or the American Red Cross is required within 90 days of employment.
Compensation:
$231,876 - $331,251
