Job Summary:

We are currently seeking a dedicated and experienced Long-Term Services Support Coordinator to join our team and help us achieve our mission. If you are passionate about enhancing the quality of life for seniors and people of all ages with disabilities, we would love to hear from you

As a Long-Term Services Support Coordinator, you will play a crucial role in ensuring that our residents receive the best possible care and support. You will be responsible for coordinating and overseeing various aspects of long-term services, working closely with residents, families, and staff to ensure that all needs are met efficiently and empathetically.

Responsibilities:

· Resident Support: Develop and maintain individualized care plans for residents, addressing their physical, emotional, and social needs. Regularly review and update care plans in collaboration with residents, families, and healthcare professionals.

· Coordination: Act as the primary point of contact for residents and their families regarding long-term services. Coordinate with various service providers and ensure that all services are delivered as per the care plan.

· Assessment: Conduct comprehensive assessments of new and existing residents to determine their needs and preferences. Identify and address any gaps in services or support.

· Communication: Facilitate open and effective communication between residents, families, and staff. Address any concerns or issues promptly and professionally.

· Documentation: Maintain accurate and up-to-date records of care plans, service delivery, and resident progress. Ensure compliance with relevant regulations and company policies.

· Advocacy: Advocate for residents' needs and preferences, ensuring they receive appropriate and respectful care. Work to resolve any conflicts or issues that may arise.

· Training: Provide training and support to staff members regarding resident care and service coordination. Promote best practices in resident support and service delivery.

Requirements:

· ***Bachelor’s degree in Social Work, Human Services, or a related field preferred.

· Proven experience in a similar role within a senior living or healthcare setting.

· Previous experience working with people with disabilities a plus.

· Community Health Worker (CHW) certification highly preferred.

· Strong understanding of long-term care services, regulations, and best practices.

· Excellent interpersonal and communication skills, with the ability to build strong relationships with residents, families, and team members.

· Strong organizational and problem-solving skills, with the ability to manage multiple tasks and priorities.

· Compassionate and empathetic approach to working with seniors and people with disabilities (adults and children).

· Proficiency in Microsoft Office Suite and other relevant software.

· Must have reliable transportation and valid driver’s license for community outreach and home visits.

· Ability to work both independently and as part of a team.

· Professional demeanor and ability to handle confidential materials with discretion.

· Bilingual (Spanish) preferred.
