Job Summary

We are seeking a skilled Registered Nurse to join our Home Care PACE team at CarePartners. As a Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with the interdisciplinary team to establish and achieve patient goals.

Responsibilities
  • Assess patients' physical, psychosocial, and environmental needs, identifying areas for improvement and developing personalized care plans.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
  • Collaborate with the interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
  • Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services.
  • Participate in on-call rotation and emergency call, providing timely and effective responses to patient needs.
Requirements
  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Registered Nurse licensure in the state of North Carolina.
  • Associate Degree in Nursing or higher.
  • Valid driver's license.
