About the Role

CarePartners is seeking a skilled Registered Nurse Case Manager to join our Hospice Home Care team. As a key member of our team, you will play a vital role in ensuring that our patients receive high-quality, patient-centered care.

Responsibilities

  • Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
  • Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.

Requirements

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • Drivers License must be obtained within 2 years of employment start date.
  • Registered Nurse must be obtained within 2 years of employment start date.
  • Associate Degree.
