About the Role

CarePartners is seeking a compassionate and skilled Registered Nurse to join our Hospice Home Care team. As a Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, promoting comfort, dignity, and quality of life.

Responsibilities
  • Assess patient needs and develop individualized care plans to meet their unique requirements.
  • Administer medications, monitor patient responses, and document care in the patient's medical record.
  • Collaborate with medical providers, families, and caregivers to ensure comprehensive care and support.
  • Provide education and support to patients and families on disease management, treatment options, and self-care techniques.
  • Participate in interdisciplinary team meetings to discuss patient progress, address concerns, and coordinate care.
Requirements
  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Associate Degree in Nursing or equivalent.
  • Valid driver's license.
