About the Role

CarePartners is seeking a highly skilled Registered Nurse to join our team as a Home Health Clinical Coordinator. This is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to make a real difference in the lives of our patients.

Key Responsibilities

  • Coordinate the day-to-day operations of home health services for our PACE participants and families.
  • Act as a liaison between the contracted home health agency and the PACE program, ensuring seamless coordination of homecare services.
  • Utilize nursing skills to assess participants in their home environment and coordinate plans of care with appropriate resources.
  • Provide treatments and health education for participants as needed.
  • Participate in interdisciplinary team meetings to ensure compliance with competency requirements and maintain accurate medical records.

Requirements

  • Current licensure as a Registered Nurse with the North Carolina Board of Nursing.
  • Minimum of 2 years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting.
  • Experience working with frail or elderly populations is highly desirable.
  • Computer literacy with EMR and familiarity with telehealth and video consultations is preferred.
