Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled and compassionate Home Health Physical Therapist to join our team in Dover, Delaware. As a Home Health Physical Therapist, you will have the opportunity to make a meaningful difference in the lives of our clients, providing one-on-one care and support in the comfort of their own homes.
About the Role- Provide physical therapy services to adult patients of all ages, diagnoses, and acuity levels in their homes.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continuously assess and revise the care plan, collaborating with nursing and other healthcare professionals as needed.
- Educate and instruct clients, family members, or caregivers on rehabilitative care and activities to promote independent living.

- Flexible scheduling and mileage reimbursement.
- A current Delaware Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
