Job Title: Physical Therapist for Home Health Visits

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team for a part-time opportunity performing home health visits for our senior living office.

About the Role:

This position involves making home visits to clients in designated geographic territories, performing diagnostic tests and measurements, and developing and implementing individualized care plans.

Responsibilities:
  • Conduct home visits to clients in designated territories
  • Perform diagnostic tests and measurements, such as mobility and range of joints, transfer status, stability, and safety assessments
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions
  • Continually assess and revise the care plan, and participate with nursing in the multidisciplinary care plan
  • Educate and instruct clients, family members, or other representatives in rehabilitative care and activities
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided
Qualifications:
  • Current Pennsylvania Physical Therapist license
  • A minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred
