Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing high-quality home health care services to our clients in Zullinger, Pennsylvania.

About the Role:

As a Physical Therapist with BAYADA, you will be responsible for providing one-on-one care to our clients in the comfort of their own homes. You will work closely with our multidisciplinary team to develop and implement individualized care plans that promote the health, safety, and independent living of our clients.

Responsibilities:
  • Perform diagnostic tests and measurements to assess client needs and develop effective care plans.
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
  • Continually assess and revise care plans to ensure optimal client outcomes.
  • Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities necessary to promote client health and safety.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Qualifications:
  • A current PA Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
