Job Title: Home Health Physical Therapist

We are seeking a highly skilled and compassionate Physical Therapist to join our team at JonSan Home Health Inc. as a Home Health Physical Therapist. As a Physical Therapist, you will play a vital role in providing high-quality physical therapy services to patients in their homes, promoting independence, and improving their overall quality of life.

Responsibilities:
  • Assess and evaluate patients' physical therapy needs, developing individualized treatment plans to achieve their goals.
  • Conduct thorough evaluations of patients' home environments to identify potential safety hazards and develop strategies to mitigate them.
  • Provide physical therapy services, including exercises, stretches, and other interventions, to help patients regain strength, mobility, and function.
  • Collaborate with patients, families, and caregivers to educate them on proper use and care of therapeutic appliances and equipment.
  • Document patients' progress, including treatment plans, progress notes, and discharge summaries.
  • Participate in case conferences and multidisciplinary team meetings to discuss patients' care plans and coordinate services.
  • Supervise and train Physical Therapy Assistants and other staff members as needed.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure high-quality patient care.
Requirements:
  • Master's or Doctoral degree in Physical Therapy from an accredited program.
  • Current Physical Therapy License in the state of Texas.
  • At least two years of experience as a Physical Therapist, preferably in home health care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to lift, bend, and move patients as needed, with or without reasonable accommodation.
