Nurse Care Coordinator Specialist

About the Role:

As a Nurse Care Coordinator Specialist, you will be responsible for providing comprehensive care coordination services to patients and their families. Responsibilities:

  • Assess patient health status and develop personalized care plans
  • Collaborate with healthcare professionals to coordinate patient care
  • Educate patients and families on disease prevention and management
  • Administer medications and treatments as prescribed by physicians
  • Document patient care and maintain accurate medical records

Requirements:

  • Bachelor's degree in Nursing or related field
  • Current RN licensure in good standing
  • Minimum 2 years of experience in nursing or related field
