About the Role

We are seeking a highly skilled Registered Nurse to join our team as a Home Health Clinician. This is an exciting opportunity for a dedicated and compassionate individual to make a meaningful difference in the lives of our clients.

As a Home Health Clinician, you will be responsible for providing high-quality patient care in the comfort of their own homes. You will work closely with our multidisciplinary team to develop and implement personalized care plans that meet the unique needs of each client.

Key Responsibilities
  • Conduct thorough assessments of clients' physical and emotional needs
  • Develop and implement comprehensive care plans in collaboration with our interdisciplinary team
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
  • Collaborate with clients' families and caregivers to ensure seamless transitions and coordination of care
  • Document all interactions, interventions, and outcomes in accordance with our electronic medical record system
Requirements
  • A current license as a Registered Nurse in Vermont
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Wound Care Certified
  • Prior home care experience strongly preferred, but not required
  • Graduation from an accredited and approved nursing program
