About the Role

As a Home Health Registered Nurse with Interim HealthCare, you will have the opportunity to provide one-on-one care to patients in the comfort of their own homes. You will work independently, making decisions and taking charge of patient care, while also being supported by a team of experienced professionals.

Responsibilities
  • Complete initial and ongoing comprehensive assessments of patients
  • Coordinate patient discharge from services
  • Provide care according to patient plans of care
  • Participate with patients in clarifying mutually agreed upon goals and incorporate these goals into patient plans of care
  • Conduct home visits as assigned, assessing patient conditions, progress toward goals, providing patient care, teaching, and other skilled interventions as ordered
Requirements
  • Current Registered Nurse (RN) license in the state of Arizona, in good standing
  • CPR Certification (demonstration course required)
  • Valid Arizona Fingerprint Card
  • One (1) Home health experience preferred but not required
