Role Summary

The Patient Care Coordinator acts as a vital link among patients, families, hospital personnel, and physicians, ensuring a positive patient experience through effective communication and support. This role involves managing unit-receptionist tasks and executing essential clerical duties within a nursing unit.


Key Responsibilities
  • Facilitates unit-receptionist functions, including managing communications within and outside the unit, addressing customer inquiries, and coordinating patient room assignments and transfers.
  • Oversees supply ordering based on established par levels and promptly answers unit telephones, addressing caller needs efficiently.
  • Monitors and initiates maintenance requests for equipment and facility repairs.
  • Maintains daily unit documentation and logs, ensuring accurate record-keeping.
  • Conducts hourly rounds to assess patient needs, delivering personal care items and communicating clinical requests to nursing staff.
  • Assists with patient transport and discharge processes, ensuring a smooth transition for patients.
  • Coordinates clinical data collection and documentation, ensuring compliance with hospital policies.
  • Maintains patient records, ensuring all necessary forms and documentation are complete and accessible.
  • Communicates unacknowledged physician orders to nursing staff and enters patient billing information as required.

Patient Demographics

The Patient Care Coordinator must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served, which may include:

  • Neonates (0-4 weeks)
  • Infants (1-12 months)
  • Pediatrics (1-12 years)
  • Adults (18-64 years)
  • Geriatrics (65 years and older)

Qualifications

Education: Minimum requirement is less than a High School Diploma; a High School Diploma or GED is preferred.

Licensing/Certification: Completion of a Medical Terminology course is required within one year of hire. Heart Saver certification is preferred.

Experience: Previous experience in a hospital setting is required, with prior unit clerk experience preferred.


Skills and Abilities

The ideal candidate will possess strong communication and customer service skills, along with the ability to manage challenging situations effectively. Computer literacy, including proficiency in charting and documentation, is essential.


Work Schedule

This is a part-time position, scheduled for 24 hours per week, with variable shift times. The role includes scheduled shifts every other weekend and holiday.
