Under the Department of Pharmaceutical Services, the Cardiology Ambulatory Pharmacist provides pharmaceutical care to patients at UCSF Ambulatory Care Clinics. This role involves a wide range of pharmaceutical care functions to ensure that drugs prescribed for and used by patients are safe and effective.
Key Responsibilities- Obtain medication histories and review medication profiles
- Provide drug information to patients and healthcare providers
- Collaborate with clinicians on drug selection and therapeutic plans
- Ensure access to medications and educate patients on proper use
- Supervise pharmacy technicians to facilitate insurance authorizations
- Doctor of Pharmacy degree from an accredited college or university
- California Board of Pharmacy license or eligibility
- Completion of a PGY1 Pharmacy Residency or equivalent experience
- Excellent interpersonal and communication skills
- Computer skills for clinical patient data medication order entry
- Completion of a PGY2 Ambulatory Pharmacy Residency or equivalent experience
- Board certification (e.g., BPS)
- Experience with navigating billing options for pharmacist care
- Experience precepting and evaluating pharmacy learners
