Operational Staff Pharmacist II

The Department of Pharmaceutical Services at the University of California provides a comprehensive range of services for patients cared for by the Medical Center and its providers. These services include drug procurement, storage, and security, as well as safe, accurate, and timely preparation and distribution of drugs. The department is affiliated with the UCSF Department of Clinical Pharmacy, an academic department within the School of Pharmacy.
The Operational Staff Pharmacist II provides pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System. This includes obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and healthcare providers, and collaborating with clinicians on drug therapy plans. The pharmacist will also be responsible for ensuring access to medications, educating patients on proper medication use, and monitoring for potential interactions and adverse reactions.
The ideal candidate will possess a Doctor of Pharmacy degree and be licensed to practice pharmacy in the State of California. Completion of an ASHP-accredited PGY1 Pharmacy Residency or at least 3 years of inpatient/operational pharmacy experience is preferred. The pharmacist will be part of an interprofessional team consisting of physicians, advanced practice providers, nurses, and other professionals.
