Clinical Staff Pharmacist II

The Department of Pharmaceutical Services at the University of California, San Francisco (UCSF) provides a comprehensive range of services for patients cared for by the Medical Center at UCSF and its providers and payors.
As a Clinical Staff Pharmacist II, you will be part of an interprofessional team consisting of physicians, advanced practice providers, nurses, and other professionals. Your primary responsibility will be to provide pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System.
You will be responsible for a wide range of pharmaceutical care functions, including obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and healthcare providers, collaborating with clinicians on drugs of choice and therapeutic plans, ensuring access to medications, educating patients on the proper use of medications, and ensuring that patients understand cautions, adverse reactions, and interactions related to their therapy.
Key Responsibilities- Provide pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System
- Obtain medication histories, review and assess medication profiles, and provide drug information to patients and healthcare providers
- Collaborate with clinicians on drugs of choice and therapeutic plans
- Ensure access to medications and educate patients on the proper use of medications
- Ensure that patients understand cautions, adverse reactions, and interactions related to their therapy
To be considered for this role, you must have a Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy. You must also possess a current license in good standing to practice pharmacy in the State of California and current certification for Basic Life Support (BLS) from the American Heart Association.
Preferred qualifications include completion of an ASHP-accredited PGY1 Pharmacy Residency or at least 3 years of inpatient/acute care services pharmacy experience. Experience participating in pediatric and adult emergency situations is also desirable.
