Job Summary:

As a Palliative Care Nurse Practitioner, you will play a vital role in overseeing the clinical care and treatment of patients and families. You will consult with attending physicians and provide clinical direction to the Interdisciplinary Group to ensure quality care.

Key Responsibilities:
  • Assist in overseeing the clinical care and treatment of patients and families
  • Consult with attending physicians and provide clinical direction to the Interdisciplinary Group
  • Provide comprehensive knowledge of pharmacology, palliative care, and end-of-life care
  • Develop and implement individualized care plans for patients and families
  • Collaborate with healthcare teams to ensure seamless care transitions
Requirements:
  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State license as a Registered Nurse and as an Advanced Practice Nurse
  • Current Driver's License
  • Two years experience as an APN in a clinical care setting preferred, hospice or home health preferred
  • Comprehensive knowledge of pharmacology, palliative care, and end-of-life care
  • Good interpersonal skills
Working Conditions:

You will work in various environments, including patient homes, nursing homes, assisted living facilities, hospitals, and inpatient units. You may be exposed to blood and bodily fluids and infectious diseases. You must be able to work a flexible schedule and travel locally.
