Key Responsibilities:- Provide clinical direction to the Interdisciplinary Group to ensure quality care
- Consult with patients' attending physicians and provide clinical guidance
- Develop and implement care plans in collaboration with patients and families
- Collaborate with healthcare professionals to provide comprehensive care
- Conduct patient assessments and develop treatment plans
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State license as a Registered Nurse and as an Advanced Practice Nurse
- Current Driver's License
- Two years experience as an APN in a clinical care setting preferred, hospice or home health preferred
- Comprehensive knowledge of pharmacology, palliative care, and end-of-life
- Good interpersonal skills
The successful candidate will work in a variety of settings, including patient homes, nursing homes, assisted living facilities, hospitals, and inpatient units. The role requires the ability to work a flexible schedule and travel locally. The candidate should be able to work in a fast-paced environment and be able to handle multiple priorities simultaneously.
