Enhabit Home Health & Hospice seeks a seasoned Registered Nurse to deliver exceptional end-of-life care to patients and their families in private homes, residential care facilities, and nursing centers. As a Hospice RN, you will play a critical role in developing and implementing personalized care plans, collaborating with healthcare professionals, and maintaining accurate records and documentation.
**Core Responsibilities:**
- Develop and implement individualized care plans that address the unique needs and preferences of patients and their families
- Conduct regular assessments and evaluations to identify changing needs and preferences
- Collaborate with multidisciplinary teams to ensure seamless coordination of care
- Supervise and mentor licensed vocational nurses and nursing aides as needed
- Maintain accurate and detailed records and documentation
- Stay up-to-date on industry developments and best practices through ongoing education and training

**Requirements:**
- Current licensure as a Registered Nurse in the state of employment
