Job Summary:

The Bay Area Hospital seeks a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will be responsible for providing comprehensive pharmaceutical care services to patients across various disease states.

Key Responsibilities:

  • Develop and implement medication therapy plans to ensure safe and effective treatment of patients.
  • Collaborate with multidisciplinary care teams to monitor and assess medication use, and provide clinical interventions as needed.
  • Stay up-to-date with the latest evidence-based practices, policies, and regulatory standards to ensure optimal medication therapy.
  • Provide education and training to healthcare professionals on medication use and management.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Doctor of Pharmacy (Pharm.D.) degree or Bachelor of Science in Pharmacy (B.S. Pharm) degree from an accredited institution.
  • Current Oregon Pharmacist license.
  • American Heart Association Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
  • Minimum of 3 years of relevant hospital experience.
