Chamber Cardio is seeking a skilled Practice Liaison to join our care team. As a key member of our team, you will provide practice coordination services to meet the medical, behavioral health, and social needs of our members while promoting high-quality care.

Responsibilities:

* Collaborate closely with Care Coordinators to ensure seamless care delivery
* Build strong relationships with physicians, social service providers, health plans, and office staff to ensure effective communication and coordination
* Utilize the Chamber Platform to identify opportunities for connection and alignment among stakeholders
* Provide information to providers regarding benefits, care plans, service providers, and access protocols
* Refer patients to community resources and social services as needed
* Monitor provider performance and educate offices on best practices

Requirements:

* High School diploma or equivalent required
* Minimum of 2 years of relevant healthcare experience in a clinical setting
* Preferred qualifications include knowledge of medical terminology, detail-oriented skills, and proficiency in Microsoft Office Suite products
* Ability to exercise proper phone etiquette and navigate computer software systems
* Team-oriented with excellent communication and organizational skills
* Spoken and written fluency in English; bilingual a plus

Workstyle:

* This is a hybrid position

Work Location:

* Must reside in Washington D.C. or Maryland

Travel:

* Occasional travel to physician offices

Work Days/Hours:

* Monday - Friday, 8:00 am - 5:00 pm EST
