Nurse Care Coordinator for Developmental Disabilities

**Job Summary:** We are seeking a highly skilled and compassionate Registered Nurse to join our team as a School Registered Nurse. This is a full-time position with a school schedule (8:15-3:15). The successful candidate will work closely with our interdisciplinary team to provide high-quality healthcare services to individuals with developmental disabilities.
About the Role:
- This role involves coordinating and following through on provisions of health care services for assigned residences, including conducting training for staff in medical administration, personal hygiene, basic first aid, dental hygiene, infection control, and adaptive equipment.
- You will be responsible for handling the control of communicable diseases and infections through identification and assessment, reporting to consulting physicians, and implementing appropriate protective and preventive measures and staff training in regard to infection control.
- Develop and supervise the implementation of each resident's written health care plan, which should be reviewed and revised as needed, in accordance with instructions from a physician and assessment.
- Integrate therapeutic intervention techniques into programs or homes of persons served through consultation with parents/clients and staff at team meetings, case conferences, or informal contacts.
- Develop a written plan for each resident to provide nursing services as part of total habilitation programs.
- Participate in interdisciplinary team meetings and annual reviews as required by each division.
- Monitor required staff health records and provide tests in accordance with regulatory agency requirements.
Requirements:
- Licensed as a Registered Nurse in the state of New York.
- At least one year of experience as a Registered Nurse in the delivery of community-based health services to individuals with developmental disabilities, preferably to children.
Salary and Benefits:
